Persist Care and Support Services Limited is a domiciliary care provider committed to delivering short-term, person-centred reablement and domestic support services. Our primary aim is to assist individuals recently discharged from hospital in regaining their independence and confidence within the comfort of their own homes, typically over a structured six-week recovery period. We offer support with daily living activities, including cleaning, shopping, and prescription collection. As a core service provider, we are also able to carry out Care Act assessments for adults who may present with eligible care and support needs.
